In areas with no ties to units should be selected the neighborhood in which the services are provided.
Comments: Complements at the discretion of the citizen, the view from the insertion of comments, notes, suggestions or comments on the situations experienced at the time of receipt of public service considered relevant, ie noteworthy.
Attachments: send photos and / or images at the time of the demonstration on the satisfaction or not with the provision of public services as a way of qualifying the same.
